Instructions for TransChart PATIENT Use

Quick Login

  1. Login to the system using the user name and password provided.  If you do not have a login please e-mail the Transplant Division to obtain a login acccount.  Select patient login from the menu to the left.
  2. Enter your username and press the [TAB] key.
  3. Enter your password and press the [ENTER] key.
  4. Confirm that your name and medical record is correct.  If it is correct mouse click on The above information is correct.   Otherwise click on The above information is not correct and login again.
  5. Follow the on-screen prompts to add and view your medical record.
  6. Items are color coded -  BLACK is results, MAROON is patient identifiers or useful information,  RED is instructions and warnings
  7. Holding your mouse over a particular item will display added infomation.

Security

Your information is kept secure using a complex system on computers and computer hardware.  For security reasons we will not go into detail.  In addition, encryption certificates have been obtained from Verisign.  They assure that if any information being transmitted falls in to the incorrect hands, the information cannot be read.  We are using the same technology major financial institutions use.

E-mailing OSU transplant

Because e-mail is not a secure mechanism to communicate, this mechanism should NOT be used to transmit medical information.  E-mail should be limited to technical or general questions. 

Who can see my information?

Only you and your health care providers can see your medical record. If you have given permission, your referring (non-osu) physician can also see your medical record.
